4 September 1940 - 264 Squadron, RAF Kirton-in-Lindsey – Boulton Paul Defiant – N1628 - F/O Derek K C O'Malley – Night Patrol.

Location – Northorpe 3 miles west of Kirton-in-Lindsey

Took off from RAF Kirton-in-Lindsey on night patrol. For reasons unknown the aircraft failed to climb and crashed into the ground at Northorpe 3 miles to the west of the airfield.

F/O Derek Keppel Coleridge O'Malley RAFVR – Pilot – 29 - 264 Sqn - Son of Barret Lennard Albemarle O'Malley and of Lorna O'Malley (nee Coleridge); husband of Rachel Mary Genevieve O'Malley (nee MacDonald), of Henley-on-Thames, Oxfordshire. B.A. (Oxon.) - Kirton-in Lindsey Cemetery, Lincolnshire.

Sgt Laurie Andrew Woodney Rasmussen RNZAF – Air Gunner - shown as Pilot in CWG Register – 18 – 264 Sqn - Son of Frederick Woodney Rasmussen and Annie Shaw Rasmussen, of Remuera, Auckland, New Zealand - Kirton-in Lindsey Cemetery, Lincolnshire.
